Yes, but do I get the freeze automatically (as long as I agave one available) or do I have to do something within the app?
you don’t have to do anything as far as I know, the streak saver should automagically be used if you have it available

Streak Savers
Zwift knows that riders may be unable to ride for a week or more, though, due to vacation, injury, or other factors. So in April 2025 they introduced Streak Savers to the game. These are automatically used to extend your streak if you have a week with no qualifying activities.
Streak Savers are displayed in the Companion app, next to your Week Streak score:

Can confirm from experience that the streak savers are applied automatically. I had two of them before heading on vacation for two weeks, and when I did my first Zwift ride after returning the icons for the streak savers were gone and my streak was still going (and, crucially, my Scotty was still in my pocket).
